Key Responsibilities:
- Coordinate client care plans to ensure seamless recovery and reintegration.
- Conduct thorough assessments to identify health needs and barriers to recovery.
- Partner with allied health professionals, employers, and insurers for unified care.
- Guide clients and families through the rehabilitation process with education and advocacy.
- Track client progress, adjust care plans as needed, and prepare detailed reports.

Requirements:
- Tertiary qualifications in Nursing
- Full AHPRA registration
