Key Responsibilities- Conduct thorough speech pathology evaluations utilizing appropriate assessment tools and create detailed Health Management Plans.
- Provide timely and precise assessments and interventions for both internal and external clients, ensuring alignment with team and individual practice scopes.
- Collaborate with clients and stakeholders to develop personalized care plans aimed at achieving client-centered objectives.
- Identify opportunities for quality enhancement from the perspectives of clients, quality standards, and organizational goals.
- Recognize the need for additional support services within care plans in partnership with clients.
- Facilitate necessary internal and external referrals for both group and individual interventions to promote coordinated health outcomes.
- Engage with a diverse client base, addressing a wide range of clinical presentations.
- Participate in service meetings, staff gatherings, and other organizational discussions as required.
- Ensure that the standards of speech pathology services are grounded in current best practices and adhere to the Speech Pathology Australia - Code of Ethics.
To excel in this position, you should possess:
- A tertiary qualification in Speech Pathology.
- Active membership with Speech Pathology Australia.
- Proven skills and experience in speech pathology assessment and intervention.
- The capability to work effectively in teams or independently.
- A commitment to ongoing professional development and a passion for delivering exceptional care to clients.
- Successful candidates will undergo necessary background checks and must hold a valid working with children check.
- A full driver’s license is required for travel between various sites.
- This position is classified as a Grade 2 Speech Pathologist, depending on the applicant's experience and qualifications.
