Job Description:

This role involves providing support to elderly clients in their homes and communities. The goal is to enhance their safety, promote independence, and optimize health outcomes in collaboration with a multidisciplinary team.

The successful candidate will provide high-quality physiotherapy services to clients and carers, ensuring the provision of optimal health outcomes. This will involve problem-solving skills to support safe mobility in the home and community setting without direct clinical supervision when completing in-home assessments and interventions.

Additionally, the Physiotherapist will be responsible for care planning, case management, and liaison with external stakeholders. This position functions as part of a multi-disciplinary team using a teamwork approach to provide low-intensity therapy home-based programs to frail elderly clients who are ready for discharge from sub-acute hospital care but require additional care and support.


Required Skills and Qualifications:

Applicants must have current AHPRA registration and a valid NSW driver's license. They must also meet the applicable vaccination requirements for a Category A role.

The ideal candidate will possess strong problem-solving skills, excellent communication skills, and the ability to work effectively in a team environment.
