About the TeamOur team works collaboratively to optimise functional and health outcomes and reduce hospital admissions for clients with Diabetes, Chronic Cardiac and Respiratory Conditions, and vulnerable people with other chronic conditions. We aim to meet the needs of the client and provide support and supervision to less experienced staff.
