**Job Opportunity for Home Support Specialist**

We are seeking an experienced Home Support Specialist to join our team in the Sydney Metro Area. This casual position involves visiting clients in their homes, conducting assessments, and reporting outcomes.

The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work independently with team support. Relevant qualifications in Social Science, Nursing, Allied Health or related field are desirable, but not essential.

**Key Responsibilities:**

  • Conduct face-to-face assessments of multiple Aged Care clients each day (as allocated).
  • Update electronic National Screening and Assessment Form (NSAF) online (real time) in conjunction with the client and generate a Support Plan with a reablement focus.
  • Match and refer assessed clients to appropriate Commonwealth Home Support Programme Services (and other funded aged care services).
  • Reassess existing clients where a significant change arises in the client's circumstances or needs.

**Required Skills and Qualifications:**

  • Ability to communicate effectively and put clients at ease whilst building instant rapport.
  • Must have a current unrestricted Australian Driver's Licence and registered and insured vehicle.
  • Computer literacy and reporting skills are essential.
