Role Summary

This is a case worker role focused on supporting and empowering individuals and communities in times of vulnerability.

About the Role

The purpose of this role is to prevent homelessness and help people maintain their independence by assisting them with keeping their current tenancies.

  • We provide access to relevant services, social support networks, and address issues that could lead to homelessness.
  • Tenancy Sustainment Services support individuals and families at risk of homelessness or who have recently secured housing by providing case management, life skills development, and assistance in accessing education, employment, and other services to help them maintain stable tenancies.

As a case worker, you will provide individualised support to clients to set and attain identified goals. You will collaborate with other agencies and organisations to source appropriate support, contribute to co-design processes, and adapt and innovate ways of working to optimise our response to our clients.

Key aspects of this role include:

  1. Providing case management to individuals and families at risk of homelessness;
  2. Developing and implementing individualised plans to support clients in maintaining stable tenancies;
  3. Collaborating with other agencies and organisations to source appropriate support for clients;
  4. Contributing to co-design processes to inform service delivery and improve outcomes for clients;
  5. Adapting and innovating ways of working to optimise our response to clients.
