Care Partners play a vital role in supporting individuals with diverse and complex care needs. They are part of a multidisciplinary team working towards healthy ageing and wellness goals for their clients.

Key responsibilities include delivering person-centered case management support to participants receiving home care services, assessing and documenting participant needs and goals, collaborating with health professionals and suppliers involved in care, and working with participants and their support network to achieve goals.

The ideal candidate will have a Certificate IV in Ageing Support, experience in supporting ageing individuals, excellent assessment, documentation, and care plan implementation skills, exceptional negotiation and interpersonal skills, and the ability to thrive in a fast-paced environment. A tertiary qualification in aged care, health, or allied health is also desirable.
