As a Dementia Support Clinician, you will play a vital role in providing comprehensive support to individuals with dementia and their families. This rewarding position offers the opportunity to make a meaningful impact while working in a collaborative team environment.

- Provide home visits and community-based support services
- Work collaboratively with healthcare professionals to ensure seamless care transitions
Requirements:
- Current registration with AHPRA as a Registered Nurse
- Proven clinical experience in dementia or aged care case management
- Strong communication and interpersonal skills
- Ability to work autonomously and as part of a multidisciplinary team
