Requirements
  • Tertiary qualifications in Nursing or Allied Health (with relevant registration)
  • Previous case management experience, particularly in community and aged care settings
  • Experience conducting HPC/STRC assessments and applying goal-oriented, outcome-focused, and holistic approaches
  • Demonstrated ability to develop and maintain strong networks with internal and external stakeholders
  • Current driver's license and willingness to travel throughout the region
  • COVID-19 vaccination
