The role involves assessing and evaluating patient care needs, developing treatment plans, and performing interventions to promote optimal well-being. You will work independently, making decisions that benefit patients while continuing education opportunities help you grow professionally.
Your responsibilities will include evaluating patient functional status, body systems, and physical therapy needs, consulting with physicians, and observing patient responses to treatment. You will also instruct healthcare teams, patients, and families about treatment plans, limitations, and progress.
Additional responsibilities include completing progress notes, submitting evaluations, participating in staff meetings, supervising home health aides and PTAs, and preparing discharge summaries. Maintaining up-to-date clinical knowledge is essential for this role.
