Role Summary

The Connect Well Program is seeking a Health Link Worker to coordinate integrated healthcare for women and children in our Nowra Crisis Accommodation.

Key Responsibilities:
  • Coordinate healthcare services for women and children residing in the accommodation.
  • Work in partnership with GP and nursing care to deliver on-site services.
  • Develop and maintain relationships with community service providers to ensure comprehensive support.
Requirements:

This role requires strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders. A background in health or social work is highly desirable.
