Nurse Practitioner - Patient Care Coordinator Role


About the Job

Provide expert patient care coordination services to ensure seamless transitions and excellent outcomes for our patients.

Utilize your nursing expertise to educate patients on pre-operative admissions processes, ensure timely preparation for theatre, and maintain accurate records of patient information.

Collaborate with multidisciplinary teams to achieve shared goals and provide exceptional customer service.

Work effectively in a fast-paced environment, prioritizing tasks, managing competing demands, and maintaining high levels of professionalism.


Responsibilities
  • Develop and implement effective patient care plans to meet individual needs.
  • Coordinate discharge planning to ensure smooth transitions between care settings.
  • Maintain accurate and up-to-date patient records, including medical history, treatment plans, and test results.
  • Communicate effectively with patients, families, and healthcare teams to address concerns and improve outcomes.
  • Contribute to quality improvement initiatives, identifying areas for growth and implementing evidence-based practices.

Requirements
  • Current registration as an Enrolled Nurse or Registered Nurse with AHPRA.
  • Minimum 2 years' experience in acute care settings, preferably in a leadership or coordinator role.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work effectively in a team environment, prioritize tasks, and manage competing demands.
