Home Health Physical Therapist

About the Role:

We are seeking a skilled and dedicated Physical Therapist to deliver exceptional patient care in a home health setting. As a full-time employee, you will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.

Key Responsibilities:
  • Evaluate patient functional status, body systems, and physical therapy needs
  • Develop and implement individualized treatment plans
  • Provide services within scope of practice defined by state laws and guidelines
  • Collaborate with healthcare teams to achieve optimal patient outcomes
Requirements:

To succeed in this role, you will need:

  • Current Physical Therapy licensure in the country
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to reliable transportation
