Clinical Expert: Home Health Physical Therapist

Job Title:
Home Health Physical Therapist
- A skilled physical therapist with expertise in assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
Responsibilities:
- Evaluate the patient's functional status, body systems as required for CMS documentation, and physical therapy needs by conducting comprehensive assessments and developing individualized treatment plans.
- Provide evidence-based physical therapy services within the scope of practice defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
- Monitor and report the patient's response to treatment and any changes in condition to the nurse supervisor and/or physician.
- Treat patients to relieve pain, restore or maintain function using various modalities such as exercises, manual therapy, and education.
- Inform and instruct healthcare team members, patients, and families about the plan of care, limitations, and progress through effective communication and collaboration.
- Visit patients within the service area and other nearby agencies as needed to provide timely and comprehensive care.
- Document progress notes for each visit and submit according to policy to ensure accurate and up-to-date records.
- Develop and implement the PT plan of care within state guidelines, reporting outcomes within 24 hours to facilitate continuous improvement.
- Participate in staff meetings and committees as requested to contribute to the development and implementation of departmental policies and procedures.
- Supervise home health aides and PTAs as per policy and regulations to ensure quality care and compliance with standards.
- Participate in weekend rotations as needed to meet the demands of the service area and ensure continuity of care.
- Prepare discharge summaries and patient condition reports every 60 days to facilitate smooth transitions and ongoing care.
- Provide in-service education as requested to enhance the knowledge and skills of healthcare team members.
- Comply with therapy standards of practice and maintain up-to-date clinical knowledge and licensure to provide optimal care and adhere to professional standards.
- Perform other duties as assigned to contribute to the overall success of the organization.
