Key Responsibilities
  • Develop and implement comprehensive mental health care plans for older persons, working closely with multidisciplinary teams and community service providers.
  • Provide expert consultation and liaison services to general practitioners, other health professionals, and community service providers to ensure seamless care coordination.
  • Contribute to the development and implementation of mental health policies and procedures, ensuring alignment with best practice and evidence-based guidelines.
  • Collaborate with other health professionals and service providers to identify and address mental health needs of older persons in our community.
Requirements
  • Hold a degree or equivalent qualification in Psychology, Occupational Therapy, or Social Work, and registration with the relevant professional association.
  • Have a strong understanding of mental health principles, policies, and procedures, with a focus on older persons.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including older persons, families, and community service providers.
  • Ability to work independently and as part of a multidisciplinary team, with a strong commitment to providing high-quality, consumer-centered care.
