Job Description

As a skilled Registered Nurse, you will be part of a pioneering service dedicated to delivering high-quality, person-centred care to patients with complex health needs. You will work in a dynamic clinic setting, providing expert nursing care including assessment, clinical interventions, and care coordination.

Our team is committed to early intervention, restorative care, and effective care coordination, ensuring seamless and integrated patient care. You will collaborate closely with a broad network of partners to support clients with complex health needs and contribute to quality improvement initiatives.

This role offers the opportunity to make a tangible difference by helping people receive high-quality care in the community and reducing reliance on hospital services.

Required Skills and Qualifications
  • Registered or eligible for registration with the Nursing Board of Australia, holding or eligible to hold a current practising certificate
  • Minimum 3 years' post-registration experience, with proven competence in nursing practice and clinical assessment relevant to the role
  • Strong communication, problem-solving and negotiation skills, with the ability to adapt to changing environments and manage competing priorities
  • Demonstrated leadership in mentoring junior staff, supporting discharge planning for complex patients, and contributing to quality improvement initiatives
