Clinical Care Manager

We are seeking a highly skilled and experienced Clinical Care Manager to join our team at Be Recruitment. As a Clinical Care Manager, you will be responsible for developing and managing the needs of our HCP clients through comprehensive assessments and the creation of holistic and goal-oriented care plans.
You will be responsible for managing the budgets of each client and providing advice to clients on utilizing funding to achieve their health and well-being goals. You will also build relationships and rapport with your clients and their families, providing ongoing case management and completing all necessary documentation to ensure compliance with policies and procedures.
Maintaining oversight over clients' outcomes and changes in circumstances, you will conduct care plan reviews to ensure clients receive the highest level of care. About YouTo be successful in this role, you will need to have a strong background in community aged care, with experience as a Clinical Care Coordinator, Clinical Case Manager, or Registered Nurse. You will also need to be AHPRA Registered and have a NSW Driver's license and own vehicle.
You will be able to investigate, manage, and resolve complaints, and respond to incidents, as well as provide training to others to develop their skillset in this area.
