The role of Practitioner, Sustaining Tenancies involves providing person-centred early intervention/prevention support to individuals and families at risk of homelessness.

About the Job
  • This position delivers essential services to prevent and post-crisis support to clients to maintain their housing for as long as needed.

Key responsibilities include:

Job Responsibilities
  • Developing and implementing case plans using a Trauma-Informed approach
  • Assisting clients in securing and maintaining housing, promoting independence and community connection
  • Maintaining accurate client records and fulfilling data collection requirements
  • Adhering to quality standards and encouraging feedback through surveys
  • Working within the organization's service framework emphasizing self-determination and social justice
  • Supporting the implementation of evidence-based practices and promoting efficiency
  • Fostering positive team relationships and actively participating in meetings
  • Providing peer support and contributing ideas for process improvement

To be successful in this role, you will require:

Required Skills and Qualifications
  • A degree in Social Work, Human Services, or a related field, and/or equivalent practical experience in the public and community housing sector
  • A current National Police Check
  • A current Blue Card (Working with Children)
  • A current Queensland Driver Licence and access to own reliable vehicle
  • An ability and willingness to travel within and around Toowoomba and Southern Downs service region
Your Qualities
  • Proven experience in case management and assessment, particularly within a Trauma Informed Framework
  • Familiarity with early intervention/prevention strategies, including support for families, individuals at risk of homelessness, and vulnerable populations
  • Knowledge of the public and community housing sector, with an understanding of housing, mental health, substance abuse, and domestic violence issues
  • Excellent written and verbal communication skills, with the ability to negotiate and influence effectively
  • A collaborative mindset with the ability to build strong relationships with diverse stakeholders
  • A solution-oriented approach, capable of working independently while also contributing effectively to a team
  • Proficiency in Microsoft Office suite; willingness to learn and adapt to new software and systems
