Our organization seeks a skilled Social Support Worker to fill a parental backfill position.

We are looking for an individual who can act as a helper for dialysis patients and their families, providing essential support and care. The ideal candidate will possess strong stakeholder engagement skills, empathy, cultural sensitivity, and professionalism.

The successful applicant will work collaboratively with our teams, including Social Support, Elder Care, and Clinical Services, to deliver high-quality support to clients. They will also be responsible for personal shopping and banking, attending appointments and meetings, and visiting family members and participating in social and cultural activities.

To succeed in this role, you will need to be flexible, solutions-focused, and passionate about health equity. If you have experience working independently across complex, remote, and cross-cultural environments, we encourage you to apply.

About This Role
  • Act as a social support worker for dialysis patients and their families.
  • Provide practical support for clients with complex needs as directed by the Social Support Coordinator.
  • Work collaboratively with the Social Support, Elder Care, and Clinical Services teams.
Key Responsibilities
  • Personal shopping and banking.
  • Attending appointments and meetings (medical, housing, Centrelink, legal).
  • Visiting family members and participating in social and cultural activities.
