Job Title

We are seeking a compassionate and experienced Support Professional to join our team at Hearth, a NDIS registered provider. The ideal candidate will have professional or lived experience supporting someone with a disability and an understanding of the disability sector and person-centred support.

Key Responsibilities:

  • Support individuals in accessing their community and participating in recreational activities
  • Provide personal care assistance such as showering, toileting, and hygiene routines
  • Assist with manual handling and the use of wheelchairs & hoists
  • Transport individuals to appointments and engage them in social activities
  • Perform light domestic duties such as meal preparation and cleaning
  • Offer behavioural and mental health support to individuals

Requirements

The successful applicant will possess:

  • An understanding of the disability/community services sector and person-centred support principles
  • Transferable life/work skills along with a caring and compassionate approach
  • A valid driver's license and a registered vehicle
  • Completion of a NDIS Worker Screening Check
  • Legal entitlement to work in Australia
  • Excellent communication skills
  • A genuine interest in promoting the wellbeing and inclusion of people with disabilities
