Job Title: Community Connection Specialist

We are seeking a passionate and compassionate Community Connection Specialist to join our team. As a key member of our care community, you will play a vital role in enriching the lives of residents through engaging activities and meaningful connections.

About the Role:

  • Plan and deliver activities that cater to diverse interests and needs of residents, fostering a sense of belonging and connection within the community.
  • Assist with daily living tasks, offer emotional support, and gather feedback to ensure activities meet the needs of residents.
  • Collaborate with the care team to plan activities, document progress, and escalate concerns as needed.
  • Maintain a tidy environment and adhere to infection control protocols, ensuring the well-being of residents.
  • Assist with meal preparation, service, and monitoring resident preferences, promoting a positive dining experience.

Requirements:

  • A heart of gold and a can-do spirit, with excellent communication skills and previous experience in caring for elders.
  • Creative spark and ability to think outside the box when planning activities and engaging residents.
  • Prioritize building relationships and trust with residents, families, and colleagues, fostering a supportive and inclusive environment.
  • Demonstrate empathy and understanding when supporting residents with their physical and emotional needs.
