  • This role is crucial for connecting consumers with services and supports, facilitating care navigation and referral support.

Key responsibilities include working collaboratively with consumers to connect them with services and supports, facilitating person-centred and integrated care, providing care navigation and referral support, and collaborating with other members of the team to maximise care outcomes for consumers.

Required skills and qualifications include a tertiary qualification in a relevant discipline or extensive experience in mental health, health, community services or social sciences, as well as a demonstrated understanding of the health and wellbeing needs of consumers with mental health and/or substance use problems.
