We are seeking a dedicated and skilled healthcare professional to provide high-quality clinical care to clients in their homes.

Key Responsibilities:

  • Clinical in-home services including care plan assessment, medication management, wound care, and chronic disease management
  • Supervision of non-clinical team members in a community setting
  • Intake processing, clinical care planning, and reporting
  • Conducting client care assessments with a focus on Consumer Directed Care
  • Coordination of quality person-centred care according to client needs and available resources
  • General administration for documentation related to client care

Requirements:

  • Australian National Police check
  • Professional Indemnity Insurance
  • Valid Driver's License and reliable vehicle
  • Smart mobile phone (iPhone/android)

Preferred Skills:

  • Passion for delivering high-quality patient-focused care in a community setting
  • Cert IV in Training and Assessment (highly desirable)
  • Able to work autonomously or within a team environment
  • Excellent time management and organisation skills
