The OpportunityWe are seeking a Clinical Care Coordinator to join our team in one of our residential aged care homes in the Hunter region of New South Wales. As a Clinical Care Coordinator, you will be responsible for the clinical care of residents, building strong relationships with residents and families, and ensuring their clinical care requirements are met.
This role requires a passion for working in aged care, strong clinical knowledge and skills, and excellent communication skills. You will work closely with our Facility Manager and other healthcare professionals to deliver high-quality care and services.
Requirements- AHPRA Nursing Registration with experience in residential aged care
- Passion for working in aged care
- Understanding and knowledge of quality aged care standards
- Strong clinical knowledge and skillset
- Excellent communication skills, including written, verbal, and computer literacy
- Ability to work with key stakeholders and build strong networks
