About the Role

This position involves working closely with families at risk of homelessness, offering a holistic approach to address their complex needs. The ideal candidate will possess strong communication and interpersonal skills, enabling them to build trust and rapport with clients.

The Assertive Case Worker will be responsible for conducting comprehensive assessments, developing individualized care plans, and coordinating services to support families in achieving stability and self-sufficiency.

Key Responsibilities:
  • Engage with families in crisis, providing assertive outreach and intensive case management.
  • Conduct thorough assessments of housing, financial, safety, and wellbeing needs.
  • Develop and implement client-centered care plans in collaboration with families.
  • Support families in accessing crisis, transitional, and long-term accommodation options.
  • Provide activity-based work and daily living skills support based on established care plans and risk assessments.
  • Assist with applications, referrals, and advocacy across various systems, including housing, Centrelink, mental health, and legal services.
  • Offer support or linkages to external services addressing child/children's wellbeing within the family structure.
  • Respond sensitively to the effects of trauma, intergenerational disadvantage, and cultural identity.
  • Support parents in enhancing parenting capacity, routines, and household stability.

This role requires a deep understanding of the complexities surrounding homelessness and a commitment to delivering high-quality services that promote positive outcomes for families.
