Clinical Care Coordinator

As a Clinical Care Coordinator at Estia Health, you will play a vital role in delivering exceptional care and support to our residents in one of our 70+ homes. We are a place where everyone belongs, with a purpose to enrich and celebrate life together.

As a Clinical Care Coordinator, you will be a leader to the team and a comfort to our residents. Your key responsibilities will include:
- Driving a continuous improvement culture
- Acting as a clinical resource for staff, offering clinical education
- Focusing on clinical service delivery, compliance, and audit processes
- Care planning in line with new standards
- Taking pride in your clinical documentation management skills
You will be someone who is empathetic and has previously worked in an aged care home. You will also have:
- A real desire to make a difference in people's lives
- Leadership experience in clinical care
- A nursing degree and current AHPRA registration
- Solid documentation skills and knowledge of ANAAC
