Role Summary:
\We are seeking a compassionate and skilled nurse with a strong interest in mental health to deliver holistic, person-centred care to older adults experiencing health challenges, chronic conditions, and complex needs.
\This role involves working as part of a multidisciplinary team to provide early intervention, health education, and recovery-focused support within clients' homes and community settings.

- Provide direct clinical nursing care to clients in their homes in line with best practice nursing procedures\
- Conduct clinical assessments, dressings, medication assessment and catheter changes\
- Develop holistic nursing care plans in collaboration with client, carer, Care Advisor and Allied health where relevant\
- Provide safe, quality nursing practice to clients with specific or complex needs (e.g. wound care, palliative care, education and support)\
- Liaise with all parties involved in the care of the client to maintain holistic health care and wellbeing\
- Ensure all documentation is accurate and completed in a professional and timely manner\

- A genuine passion for providing high quality care to the elderly\
- Current AHPRA registration following completion of a Bachelor of Nursing or equivalent\
- Minimum 3 years post-graduate nursing experience\
- Experience in clinical assessments\
- Initiative and the ability to work independently\
- Strong computer skills, especially Microsoft Office products and ability to learn new systems\
- NSW Driver's license and a reliable vehicle\
