Job Description

The Case Manager is responsible for providing high-quality and effective support services for young people in intensive therapeutic care (ITC). The primary focus is on developing and implementing tailored case plans to achieve permanency and desired outcomes for young people and their families.

This will be achieved through a dynamic and collaborative approach, emphasizing ongoing analysis, decision-making, and record-keeping to ensure the young person's needs are identified and met.

The Case Manager works closely with ITC Coordinators, Therapeutic Specialists, House Managers, and other ITC staff to provide a comprehensive therapeutic response that supports young people in healing from trauma and reaching their full potential.

The Case Manager will deliver therapeutic support services within a trauma-informed framework and collaborate with the ITC team to implement strategies and plans for individual young people addressing presenting issues, facilitating skill development, promoting positive behavioral outcomes, and increasing engagement in education, employment, training, and community activities.

A Bachelor's degree in social work, youth work, community services, or a relevant field is required. Experience working with young people who have experienced complex trauma and an understanding of adolescent development, trauma, attachment, and resilience theory are essential.

Demonstrated skills and experience in case management practice, preferably in out-of-home care (OOHC) or child protection, are necessary. The ability to achieve positive outcomes for vulnerable young people and their families is critical.
