The Role

We are seeking a highly skilled and experienced Roving Care Manager to join our team. As a Roving Care Manager, you will be responsible for leading and managing the delivery of person-centred care to residents in our care homes. You will work closely with our General Managers to ensure the highest standard of ongoing assessment, care planning, and evaluation, and clinical governance.

Key Responsibilities

  • Oversee daily clinical operations in care homes
  • Support and lead the care home clinical team, including coaching, mentoring, staff allocations, and delegation
  • Review clinical incidents, complaints, and changes
  • Lead compliance with AN-ACC assessments
  • Manage person-centred care planning and evaluation for residents
  • Prepare and conduct clinical governance audits
  • Liaise with allied health, medical practitioners, and other services to provide optimal resident care

Skills and Experience

  • Current AHPRA registration
  • IPC Lead Qualification
  • Bachelor of Nursing or equivalent qualification
  • Proven track record in delivering quality clinical care within an aged care or acute setting
  • Excellent clinical assessment skills and care planning capabilities
  • Highly effective communication, leadership, and relationship management skills
  • An understanding of Resident Rights, Aged Care Accreditation Standards, and Outcomes
