We are currently seeking a highly skilled Clinical Pharmacist to join our Clinical Services team. As a key member of the team, you will be responsible for providing clinical pharmacy services across various wards and departments, including inpatient and outpatient areas.

You will work closely with hospital staff and patients to deliver high-quality clinical pharmacy services, providing support and information on medication management and safety practices.

Your Key Responsibilities

  • ">
  • Compile and document best possible medication history for hospital inpatients">
  • Reconcile and clinically review prescribed medications">
  • Manage supply of inpatient and discharge medications for hospital patients and some outpatients">
  • Check prepared medications prior to delivery to wards and patients">
  • Prepare discharge medication and summaries">
  • Provide medication education and counseling for patients and carers">
  • Liaise with external facilities and community healthcare providers to ensure continuity of care for individual patients">
  • Provide medication-related information, advice, and support to the multidisciplinary team">

About You

">

To be considered for this role, you must have a Bachelor of Pharmacy and APHRA registration to practice as a Pharmacist (without restrictions). Highly regarded attributes include prior demonstrated experience in ward-based clinical pharmacy, flexibility to work all shifts, including evenings, weekends, public holidays, and on-call (with training), and the ability to work at all Mater Hospital locations.
