The Youth and Family Support Coordinator plays a vital role in achieving this mission by coordinating the implementation of key parts of the young person's care plan. This is a maximum term full-time opportunity within an established centre filled with a welcoming multidisciplinary team that fosters an environment that prioritises wellbeing and balance.

Key Responsibilities:

  • Collaboratively develop and manage care plans for young people to address their mental health and psychosocial needs.
  • Coordinate and monitor the young person's care in collaboration with internal and external stakeholders.
  • Provide individualised psychosocial support and interventions to young people, including skill development and increasing social connection.
  • Incorporate family-inclusive practices and collaborate with family members and relevant individuals to support the young person's mental health.
  • Maintain strong relationships with external stakeholders, conduct risk assessments, and ensure high-quality client records while complying with professional standards and legislative requirements.

About You:

  • You are skilled in coordinating young people's needs, including care planning, risk assessment, and psychosocial interventions.
  • You have experience working with diverse populations and collaborating with various stakeholders.
  • Tertiary qualifications and/or relevant experience in youth work or care coordination.
  • Experience in youth mental health with a recovery-focused, strengths-based approach.
  • Strong organisational, written communication, and time management skills.
  • Ability to work effectively in teams and autonomously.
  • Knowledge of local services, mental health systems, and risk management.
