The successful candidate will have the ability to work autonomously, think critically, and communicate effectively with residents, families, and healthcare professionals. You will be responsible for the total care of our residents, including their physical, emotional, and social well-being.

Key Responsibilities- Provide high-quality, person-centered care to residents, including physical, emotional, and social support
- Work collaboratively with the healthcare team to develop and implement individualized care plans
- Conduct comprehensive assessments and evaluations to identify resident needs and preferences
- Develop and maintain accurate and comprehensive records of resident assessments and care plans
- Manage an AN-ACC portfolio, including assessment criteria, documentation requirements, and funding regulations
- Stay up-to-date with changes in funding regulations and ensure compliance with aged care regulations
- Participate in quality improvement initiatives and contribute to the development of policies and procedures
- Bachelor's degree or diploma in Nursing
- AHPRA Registration with no restriction
- Strong skills in documentation and record-keeping
- Understanding of AN-ACC frameworks, including assessment criteria, documentation requirements, and funding regulations
- Previous experience with AN-ACC or ACFI funding (desirable)
- Familiarity with aged care regulations and compliance requirements
- A valid NDIS Workers Screening Clearance
