Job Summary

We are seeking an experienced and passionate Senior Physical Health Nurse to join our Merri-bek Community Mental Health team in Coburg. As a Senior Physical Health Nurse, you will play a key role in leading our Physical Health Program, working closely with our multidisciplinary team to provide comprehensive care to consumers with complex mental health needs.

Key Responsibilities

  • Lead the development and implementation of our Physical Health Program, working closely with our team to identify key gaps in provision of physical health assessment and intervention.
  • Collaborate with our Physical Health RPN3 to support the team in provision of metabolic monitoring and physical health assessments and interventions.
  • Develop and maintain relationships with primary healthcare providers, including GPs and community health services, to ensure seamless care coordination and follow-up for consumers.
  • Provide leadership and guidance to our team on physical health best practices and evidence-based interventions.
  • Contribute to the development of policies and procedures related to physical health care, ensuring alignment with our organization's strategic objectives.

Requirements

  • Appropriate qualifications and registration with AHPRA.
  • At least 5 years' experience post-qualification in mental health nursing.
  • Postgraduate qualification in mental health or a related field.
  • Understanding of the Equally Well Framework and its application to community mental health teams.
  • Proven leadership and management skills, with experience in leading multidisciplinary teams.
  • Ability to work autonomously and as part of a team, with excellent communication and interpersonal skills.
