Job Summary

We are seeking a dedicated and skilled professional to join our multidisciplinary health care team as a Social Worker, Rehabilitation Unit. In this role, you will have the opportunity to provide high-quality clinical services and leadership to patients/clients.

About the Role

  • You will be required to contribute to the provision of high-quality patient-centered care within a multidisciplinary healthcare team.
Key Responsibilities
  1. Provide expert social work services to patients/clients across the continuum of care, including assessment, intervention, education and support.
  2. Develop and implement comprehensive treatment plans that address the physical, emotional, social and spiritual needs of patients/clients.
  3. Collaborate with other healthcare professionals to ensure that patients/clients receive holistic and coordinated care.
  4. Contribute to the development and implementation of policies and procedures that promote best practice in social work services.
