Job Summary:
ACM Recruitment Pty Ltd is seeking a highly skilled Clinical Care Coordinator to join our team in Townsville QLD. As a Clinical Care Coordinator, you will be responsible for providing complex case management services to clients in the veteran community.
Key Responsibilities:
- Case Management: Develop and implement individualized case plans to support clients with complex mental health needs.
- Liaison and Partnerships: Establish and maintain partnerships with government departments, ex-service organizations, and community-based mental health and welfare agencies to ensure seamless service delivery.
- Community Development: Participate in community development and service promotion activities to raise awareness about mental health services and support.
- Program Evaluation and Research: Contribute to program evaluation and research activities to inform service development and improvement.
Requirements:
- Qualifications: Must hold a qualification in Psychology, Occupational Therapy, Mental Health Social Work, or Nursing with mental health experience.
- Registration: Must be registered with the Australian Health Practitioners Registration Agency (AHPRA) for Psychologists, Occupational Therapists, and Nurses.
- Accreditation: Must be eligible for membership of the Australian Association of Social Workers (AASW) and eligible to apply for Mental Health accreditation.
- Certifications: Must hold a Working with Children (WWC) and/or Vulnerable People Checks/Clearance, and a valid Driver's License.
