Healthcare Coordinator Specialist

About the Role:
This is a permanent part-time position starting with 3 days per week, with the possibility to increase to 5 days. The role involves traveling between customers' homes and providing individualized support in accordance with their support plans and/or medical practitioners' directions.
Key Responsibilities:
- Collaborate with medical practitioners and Allied Health professionals to deliver high-quality care
- Administer medication and provide complex clinical care, including palliative care, bowel care, catheter care, stoma care, oxygen therapy, and more
- Monitor and communicate on customers' well-being and report progress
- Ensure services are provided in a caring and respectful manner, adhering to Just Better Care's policies and procedures, legislative requirements, and program-specific guidelines (i.e., DVA Community Nursing)
- Provide training, coaching, and guidance to Community Support Professionals, Enrolled Nurses, and Support Staff/Case Managers as required
- Conduct skills assessments of Community Support Professionals performing complex care services as needed
- Demonstrate commitment to best practice and evidence of continued professional development
- Participate in staff meetings and attend training opportunities when required
Requirements:
- Current CPR/First Aid Certificate
- Current Federal Police Check, NDIS Worker's Screening Card, and Working with Children Check Blue Card
- Current Australian Driver's Licence
- Proof of working rights in Australia (e.g., passport, birth certificate)
- Reliable, registered, and insured vehicle (comprehensive insurance preferred)
- Smartphone with internet access
- Registered AHPRA
- Covid-19 Vaccination Certificate
Estimated Salary Range: $70,000 - $90,000 per annum, dependent on experience and qualifications. This salary range is reflective of industry standards for this level of seniority and expertise in the healthcare sector.
