The position of Home Care Nurse is an excellent opportunity to deliver high-quality clinical nursing care to clients in their own homes.
You will work autonomously but as part of a supportive team culture, collaborating with healthcare professionals to ensure holistic care and empowering clients through education and promoting health and wellbeing.
This role requires a compassionate Registered Nurse who is committed to providing exceptional patient outcomes.
Responsibilities:- Deliver evidence-based clinical nursing care in line with individual client needs and goals.
- Conduct comprehensive assessments, develop and implement personalized care plans, and provide wound care, catheter management, stoma care, diabetes management, and chronic disease management.
- Communicate effectively with clients, families, and multidisciplinary teams to ensure continuity of care and support.
- Provide education and promote health and wellbeing to clients and their carers, empowering them to take control of their health and make informed decisions.
- Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
- Police check, Working with Children check, CPR certification, and valid driver's license.
- Excellent communication and time management skills.
- Passion for delivering person-centered care in the community.
