Job Opportunity

We are seeking an experienced Mental Health Case Manager to support individuals living with mental illness in a part-time role within our Transitional Recovery Program.

This role is based in Nambour on the Sunshine Coast and involves providing case management, group work, and community development interventions to promote independence, wellness, and quality of life for clients.

The successful applicant will be responsible for:

  • Developing individualised support plans and conducting regular planning reviews;
  • Delivering group work and community development interventions to foster social connections and community engagement;
  • Maintaining accurate records and documentation in a confidential manner;
  • Communicating effectively with relevant stakeholders, including clients, families, and healthcare professionals;
  • Collaborating with other service providers to ensure comprehensive support for clients.
About You

We are looking for a dedicated professional who possesses:

  • Tertiary qualifications in a relevant field, such as social work, psychology, or nursing; or relevant experience in the community services sector;
  • Effective interpersonal and communication skills, with the ability to build strong relationships with clients and stakeholders;
  • A basic understanding of computer software, including Microsoft Office and Outlook;
  • Ability to work in a fast-paced environment, with flexibility to adapt to changing circumstances;
  • Current Australian Driver's Licence and a reliable vehicle for transportation purposes;
  • Working with Children Check (Blue Card), NDIS Worker Screening Clearance, and other relevant clearances.
