The Role: Delivering Exceptional Clinical Outcomes
As a District Nurse, you will be responsible for delivering comprehensive services to home care clients in Victoria on specified days. Key duties include:
- Conducting thorough clinical assessments and developing clinically appropriate care plans.
- Providing a range of nursing services, including medication management, wound care, diabetes management, catheter care, stoma therapy, and cognitive assessments.
- Working collaboratively with other healthcare professionals to provide multidisciplinary client care.
- Education clients and their families to optimize safe living within the community.
Essential Requirements
- Current Registration with AHPRA as a Registered Nurse.
- Availability on designated days.
- Sound clinical knowledge and skills.
- Experience working with clients who have chronic and complex health needs.
- Excellent verbal and written communication skills.
- Exceptional time-management skills and attention to detail.
