Role Overview

As a Client Care Specialist, you will have a background in Case Management and Care Planning. Your responsibilities will include visiting clients in their residences to perform assessments, develop care plans, and manage ongoing client needs.

Key Responsibilities

  • Conduct initial assessments and service planning for clients
  • Collaborate with clients to identify their needs and preferences regarding their Home Care Package, assisting them in building it according to their funding
  • Engage in care planning, continuous monitoring, and reassessment of client requirements
  • Regularly visit clients in their homes
  • Provide ongoing case management for Home Care Package and/or private clients

Candidate Profile

You are enthusiastic about advancing your career in case management. As part of a cohesive team, you will work collaboratively and supportively, demonstrating respect in all interactions.

This role allows for a hybrid work environment, with 70-80% of your time spent in the field visiting clients.

Preferred qualifications include:

  • A qualification in Case Management, or an undergraduate degree in Nursing, Allied Health, or Social Work is essential
  • Division 1 Nursing Qualification - desirable but not mandatory
  • Previous experience in case management is highly valued
  • Experience in conducting initial and annual assessments and care planning
  • Familiarity with the Home Care Package sector
