About the Role:

We are seeking a passionate health and aged care professional to join our team as a Support Worker. As a key member of our community team, you will provide support to workers and clients across the region.

To be successful, you must hold a formal tertiary qualification in nursing, such as a Bachelor of Nursing, and be registered with a relevant healthcare authority. You will also require a valid National criminal history/police check, First Aid and CPR certificate, and Working With Children's Check Card.

Experience in aged care or a related discipline is highly desirable, and you should have knowledge of the aged care industry, issues, Community Home Care, Continuous Quality Improvement, the Aged Care Quality Principles, and the Aged Care Quality Standards.

As a Support Worker, you will be responsible for building strong connections with clients and their families, conducting comprehensive client assessments, developing care plans, and providing professional health and clinical care advice and direction to our home care workers.

You will work closely with clients and their families on a regular basis, addressing any concerns and service satisfaction, ensuring all practices are consistent within policy and legislative frameworks, maintaining up-to-date client records, and interacting with various stakeholders.
