Job Opportunity: Home Support Advisor

Join our not-for-profit health care provider as a Home Support Advisor and contribute to delivering meaningful support services to individuals in their homes.

Location: Wonthaggi, Leongatha or Warragul

Job Type: Full-time or part-time, permanent

Hourly Rate:$44.5847

As a Home Support Advisor, you will play a vital role in overseeing a portfolio of clients, providing necessary support for them to safely stay at home while working towards their goals. Your diverse workload will bring new challenges each day, requiring your self-motivation and experience.

Key Responsibilities:
  • Deliver tailored support services to clients in their homes
  • Develop and implement individualized support plans to achieve client goals
  • Collaborate with multidisciplinary teams to ensure comprehensive support
Requirements:
  • Appropriate qualifications compliant with the Australian Qualifications Framework (e.g. Diploma of Case Management and/or Community Services/Nursing/ or related field)
  • A current Working with Children Check
  • A Victorian Driver's Licence
