Advanced Social Work Clinician

As a senior clinician in social work, you will be responsible for delivering high-level services to patients and their families.
Key Responsibilities:- Conduct thorough bio-psychosocial assessments to identify individualized care needs
- Develop comprehensive care plans tailored to the unique requirements of each patient
- Collaborate with interdisciplinary teams to facilitate discharge planning and advocate for patient needs
- Lead quality improvement initiatives to ensure evidence-based practice and continuous learning
This role requires an experienced social worker who possesses excellent clinical skills, strong communication abilities, and a commitment to delivering high-quality patient-centered care.
Requirements:- Bachelor's degree in social work with eligibility for membership of the Australian Association of Social Workers
- Significant experience as a level 2 or equivalent social worker, with demonstrated expertise in clinical casework, case management, and advocacy
- Strong understanding of social work principles, policies, and procedures, with ability to apply them in complex clinical settings
- Proven leadership and mentoring skills, with capacity to supervise and support junior staff members
