Job Overview

The Grade 2 Cardiology Social Worker will provide advanced social work services within the Heart Transplant and Cardiac Heart Failure Service. This includes comprehensive assessments using the Stanford Integrated Psychosocial Assessment for Transplant (SIPAT), support within the Trauma stream, and flexibility to work across multiple campuses.

Key Responsibilities:
  • Eligibility for membership of the Australian Association of Social Workers
  • Effective team collaboration and family engagement skills
  • Experience or knowledge in Family Violence, Child Protection, NDIS, or related areas
  • Knowledge of hospital social work legislation and policies
Desirable Qualifications:
  • Postgraduate qualifications in Social Work or related fields
  • Hospital-based experience
  • Proficient IT skills
