This role offers an opportunity to work in a dynamic and supportive environment, delivering case management and service coordination activities to individuals experiencing homelessness.
The successful candidate will be responsible for building strong relationships with participants, supporting them to sustain tenancies and settle into their communities.
Key aspects of the role include:
- Assertive outreach to deliver culturally safe three-year flexible support packages to participants who need assistance to maintain stable housing.
- Collaboration with community resources and services to achieve participant outcomes.
- Long-term focus on individualized goal planning and regular outreach appointments to provide preventative support and strategies that help break the cycle of homelessness.
Responsibilities
Essential Skills and Qualifications- Relevant degree qualification in a field related to community services.
- Experience in case management and navigating complex service systems.
- Passion for sustaining tenancies and ending homelessness.
- Ability to adapt to participants' needs effectively and collaborate with different services.
- Effective de-escalation and response tactics towards challenging behavior and crisis situations.
- Maximum term contract duration.
- Competitive hourly rate dependent on experience.

- Caseloads of 8 allowing quality time with participants.
