Social Worker Role

Caulfield Community Health Service provides community-based health and wellbeing services to people at all stages of life across Melbourne's south-eastern suburbs. Our focus is on delivering tailored solutions that cater to individuals' unique needs and goals.

  • This role would suit an experienced, dynamic Social Worker with a passion for community-based clinical practice.
  • Advanced Social Work services are provided for older Adult clients, including psychosocial assessment, counselling, interim case management and casework.
  • Individual and group work, advocacy, and outreach are offered.

The ideal candidate will have a Tertiary qualification in Social Work with eligibility for membership of the Australian Association of Social Workers. We seek a skilled professional with demonstrated Social Work experience at a Grade 2 level, possessing at least three years' post-qualifying experience.
