Home Care Assistant

A Home Care Assistant plays a crucial role in supporting individuals with their daily needs, enabling them to live safely and independently in their own homes.

The ideal candidate will be an experienced healthcare professional who is passionate about delivering high-quality care and making a positive impact on the lives of our customers.

Key Responsibilities:

  • Conduct thorough assessments of participants in their homes to identify their individual needs and develop personalized care plans.
  • Monitor and report on participant wellbeing, progress on care assessments, care plans, and healthcare plans.
  • Document participant goals, progress, and outcomes using person-centered strategies.
  • Perform administrative tasks such as data entry, processing invoices, and ensuring compliance with program guidelines.
  • Liaise with community-based agencies and service providers to ensure integrated and cohesive service delivery.
  • Assist participants with budgeting and monitoring their budgets in accordance with program guidelines.
  • Communicate effectively with key stakeholders, internally and externally, and participate in staff meetings and training opportunities.

Required Skills and Qualifications:

  • Advanced computer skills, including CRM software.
  • Strong administration and organizational skills.
  • Ability to multi-task and work in a fast-paced environment.
  • Strong prioritization and problem-solving skills.
  • Ability to work in a wellness model framework.
  • Demonstrated strengths in teamwork and working autonomously.
  • Previous experience in KPIs and meeting deadlines.
  • Current CPR/First Aid Certificate and satisfactory Criminal Record Check.
  • Minimum Bachelor's degree in Nursing (current AHPRA registration) or Enrolled Nurse (current AHPRA registration).
