This is a Full-Time Home Care Package Advisor role.
We are seeking an experienced and skilled professional to join our team, working on a day-to-day basis in the field of home care packages. The successful candidate will be responsible for providing expert guidance and support to participants and their families, ensuring that they receive the best possible care and services.
The ideal candidate will have a strong background in healthcare and experience working with home care packages. They will also possess excellent communication and interpersonal skills, being able to build strong relationships with participants, families, and other stakeholders.
As a Home Care Package Advisor, you will work closely with a variety of healthcare professionals, including nurses, doctors, and allied health specialists. You will be responsible for assessing the needs of participants and developing personalized care plans to meet those needs.

- Provide expert guidance and support to participants and their families;
- Assess the needs of participants and develop personalized care plans;
- Work closely with healthcare professionals to ensure seamless delivery of care services;
- Build strong relationships with participants, families, and other stakeholders;
- Ensure that all care services are delivered in accordance with regulatory requirements and organizational policies;
- Participate in ongoing training and professional development to enhance knowledge and skills;
Requirements:
- Bachelor's degree in a relevant field, such as nursing or social work;
- Minimum 2 years' experience working in the healthcare sector, preferably in a home care package setting;
- Excellent communication and interpersonal skills;
- Ability to work independently and as part of a team;
- Strong analytical and problem-solving skills;
- Proficiency in using electronic health records and other software systems;
- Current certification in CPR/AED and first aid;
- Valid driver's license and reliable transportation;
- Ability to work flexible hours, including evenings and weekends;
