Job Title: Client Liaison Coordinator

We are seeking a highly skilled and compassionate Client Liaison Coordinator to join our team. As a key member of our organization, you will play a critical role in ensuring our clients receive the best possible service.

Key Responsibilities:

  • Develop and implement client-centered service agreements that meet individual needs and preferences.
  • Conduct thorough assessments of client feedback and referrals to ensure effective management and compliance with relevant standards.
  • Monitor client budgets and costings to optimize resource allocation and achieve desired outcomes.
  • Conduct regular home visits to build strong relationships with clients and their families.
  • Ensure seamless integration of services and adherence to regulatory requirements.

Essential Requirements:

  • Relevant qualifications or experience in Disability Services or Aged Care Services, preferably in a similar role.
  • Excellent interpersonal and communication skills to foster strong relationships with clients, colleagues, and stakeholders.
  • A genuine passion for delivering exceptional care and making a positive impact on people's lives.
  • NDIS Worker Screening Check (or willingness to obtain) and a current Australian 'C' Class Drivers Licence with no restrictions.
  • Outstanding time management and organizational skills to prioritize workload and meet deadlines.
  • Proficiency in administrative tasks and ability to multitask in a dynamic environment.
