The Assertive Outreach Professional will form part of a team that works within a social model of health, providing direct interventions and conducting referrals across a range of domains.

  • An understanding of the issues and implications of homelessness on clients with complex needs

About This Opportunity:

You are an engaging outreach professional with over 3 years' experience. You hold a tertiary qualification in relevant fields and have experience working with people experiencing homelessness and marginalised populations within a harm reduction model of care.

You have a strong commitment to Social Model of health and trauma-informed care principles. You love the thought of undertaking assertive outreach work with vulnerable individuals. Your passion for achieving health and wellbeing outcomes is contagious.
